Paste your CSV exported from Ecwid here. Include only the shipping address column during export, or if you're doing this manually (why???), add one header row of

shipto_person_name	shipto_person_street_1	shipto_person_street_2	shipto_person_city	shipto_person_country_code	shipto_person_postal_code	shipto_person_state_code	shipto_person_country_name	shipto_person_state_name	shipto_person_phone

and then data rows underneath.
